Who can refer
Anyone supporting an NDIS participant can make a referral — support coordinators, LACs, hospital discharge teams, allied health professionals, guardians and families. Participants are also very welcome to contact us directly themselves.
How to refer

Please make sure the participant (or their decision-maker) knows you are referring them and agrees to it.
What to include
The more clearly we understand the situation, the faster we can respond. Helpful details:
- the participant’s name, suburb, and the best way and time to make contact,
- the supports you are looking for (for example SIL, STA, community access or support coordination),
- a short summary of goals, needs and anything time-sensitive (such as a hospital discharge date),
- your name, role and contact details, and
- confirmation that the participant or their decision-maker consents to the referral.
What not to send
Please do not send clinical records, full NDIS plans, assessments or other sensitive documents through the website form or ordinary email. If we need documents to progress the referral, we will contact you and arrange a secure, direct way to share them.
What happens after a referral
- We acknowledge your referral and let you know who is following it up.
- We arrange a conversation with the participant and the people who know them best.
- We are upfront about our current capacity and, if there is a wait, how long it is likely to be.
- If we are not the right fit, we will say so early and, where we can, suggest other options.
You will never be left wondering whether a referral landed.
How we work with you
Good support is a team effort. We work alongside coordinators, LACs, families and allied health — sharing progress, flagging changes early, and keeping the participant’s choices at the centre of every decision.
